Student Flu Vaccine Clinic

We are pleased to be able to offer an SDST student Flu Vaccine Clinic this year on Monday, October 26, 2020 from 3:30 pm until 7:30 pm.  The Student Flu Vaccine Clinic will be held at the Jay Freeze Lobby, Springfield Township High School.  There will be no cost to families. All vaccines will be the flu shot. Please click to read an important letter (Spanish)  about the upcoming clinics.  Due to limited time slots, please sign up for an appointment as soon as possible but, no later than October 22, 2020.  Please note: Due to insurance reasons, adults will not be eligible to receive the vaccine at this clinic.

How to register your child to receive the flu vaccine

1.   Download and read the Vaccine Information Statements.

2.   Print and complete the registration form (or call your school nurse if you would like a paper copy mailed to you):

Bring your completed Flu Vaccine Consent form with you to the clinic.  Students must be accompanied by a parent/guardian.

Please perform a symptom assessment just prior to leaving home and do not come to the Flu Vaccine Clinic if you have a fever, cough, shortness of breath, or are not feeling well.  Additionally, If you or your child has experienced symptoms of COVID-19 within 48 hours of the clinic or have been in close physical contact with a person who is known to have COVID-19, or you are isolating or quarantining because you may have been exposed to a person with COVID-19, you should not attend the Student Flu Vaccination Clinic. All participants are required to wear masks and practice social distancing.
