COVID-19 Vaccine
The COVID-19 vaccine, along with continuing to wear a mask, staying at least 6 feet apart, avoiding crowds, and washing your hands, is an important public health tool aimed at getting our families, communities, schools, and workplaces “back to normal” sooner. The Centers for Disease Control provides answers to some frequently asked questions about the COVID-19 vaccine and information about the benefits of getting a vaccine.
Registration for the COVID-19 vaccine is now available for everyone age 12 and over.
Montgomery County COVID-19 Vaccination Plan
The Montgomery County Office of Public Health is working closely with the Pennsylvania Department of Health in supplying COVID-19 vaccine. Their goal is for everyone to be able to easily get a COVID-19 vaccine as soon as large quantities are available.

Even after you get your vaccine, you will need to keep wearing a mask that covers your nose and mouth, washing your hands often, and staying at least 6 feet away from other people you do not live with. Vaccination Replacement Cards
Residents can get a copy of their vaccination record (but not a replacement card) by completing our Release of Records form and sending it to RA-DHPASIIS@pa.gov along with a clear copy of their Driver’s License or Photo ID. PA SIIS staff will review these items and then release vaccine records via email.
