Registration Information
Step 1: Review the Medical Requirements for school entry.
Step 2: Complete the Confidential Health History form (pdf). This is a form for the parent/guardian to complete. Please note: A copy of an ummunization record may be submitted instead of completing the lower half of the first page. Complete the Demographic Report form (pdf).
Step 3: Have your private physician complete the Private Physician‘s Exam Report (pdf). Physical examinations and dental examinations done after September 10 of the year preceding school entry meet the State Health Regulations.
Step 4: Have your private dentist complete the Private Dentist Report of Dental Examination (pdf). Physical examinations and dental examinations done after September 10 of the year preceding school entry meet the State Health Regulations.
For transfer students: If previous district’s School Health Record comes from within Pennsylvania and has documentation of a dental examination done in K/1, 3rd or 7th grade, skip this step.
Return all forms to the school nurse at your child’s school. Please contact your school nurse if you have any difficulty meeting any of these requirements.
