Re: Physical Examinations for Sports
Seventh grade students are eligible to participate in school-sponsored sports. Sports available are:
Fall:
- boys- football, soccer
- girls- soccer, field hockey and tennis
Winter:
- boys- wrestling and basketball
- girls- basketball
Spring:
- boys- baseball, tennis, track and field.
- girls- lacrosse, softball, track and field.
A complete physical is required for participation in the initial season (fall, winter, spring).
Your family physician or the school district medical director can satisfy this regulation. Many parents choose to schedule routine physical examinations during the summer break. In order to minimize duplication of required examinations, a PIAA Comprehensive Initial Pre-Preparticipation Physical Evaluation packet has been provided. If you choose to have your physician perform this examination please return the completed forms to the School Nurse by the start of the school year in September.
Remember this exam is to be done after June 1.
If you choose to have the sports examination performed by the school district medical director, all forms need to be submitted to the School Nurse by the start of school in September. Exams for 7th graders will be held at STMS during the first week of school in September. This service is provided free of charge. The medical director will also be available again in the late fall prior to the winter and spring sports seasons.
Each student must complete a PIAA Comprehensive Initial Pre-Preparticipation Physical Evaluation Packet, this includes:
- Section 1-Personal and Emergency Information
- Section 2-Certification of Parent/Guardian
- Section 3-Health History
- Section 4-Certification of Authorized Medical Examiner (to be completed
at school if school physician performing exam.)
