Immunization Requirements

Children entering school in all grades need the following vaccines:

4 doses tetanus* (1 dose on or after the 4th birthday)
4 doses diphtheria*(1 dose on or after the 4th birthday)
3 doses polio
2 doses measles** 
2 doses mumps**
1 dose rubella (German Measles)**
3 doses hepatitis B 
2 doses of varicella (chicken pox) vaccine or history of disease

Usually given as DTP or DtaP or DT or Td
**Usually given as MMR


*Children entering 7th grade need the following vaccines:

1 dose of tetanus, diphteria, acellular pertussis (Tdap)  (if 5 years have elapsed since last tetanus immunization)
One dose of meningococcal conjugate vaccine (MCV)


Pennsylavnia's school immunization requirements can be found in 28.PA.CODE.CH.23 (School Immunization).  Contact your health care provider or 1-877-PA-HEALTH for more information.  Your child will not be permitted to enter school without proper documentation regarding immunizations.

Please see your school nurse for further explanation regarding the Pennsylvania immunization law.  These requirements allow for medical reasons and religious beliefs.  If your child is exempt from immunizations, he/she may be removed from school during an outbreak.  

 


Physical and Dental Examinations


Pennsylvania law requires that children upon entry into school or transferring from out of state or out of country must have a physical examination and dental examination. This regulation also applies to students transferring into our school district from out of state, out of country, or in state where transferred School Health Records do not meet these criteria. This requirement includes children attending public, private, and parochial schools in Springfield Township.

Physical examinations and dental examinations done after September 10th of the year prior to school entry meet the State Health Regulations. We encourage you to have these procedures completed by your family doctor and dentist who are familiar with your child’s health history. Please have the appropriate forms completed and return them to your child’s assigned school in an envelope marked, “Attention School Nurse” on or before the opening of school in September.
