WORKERS' COMPENSATION INFORMATION

Instructions for Claims Reporting:

1. Call 1-800-445-6965 to report your work-related claim as soon as possible.

2. Advise your SDST Workers' Compensation Coordinator Emily Kehr at (215) 233-6000 X1011 that you have reported your work-related claim.

3. You must seek medical treatment for your claimed injury with one of the providers listed on your Posted Panel of Physicians for ninety (90) days from the date of your first visit.  Please have all doctor notes, restrictions, etc sent into the Human Resources Department as soon as possible.  Our fax number is 215-233-6025.

4. Please use the enclosed Pharmacy Sheet to fill your prescription at your local pharmacy. The Jordan Reses Company, our pharmacy management company, will send you a personalized pharmacy card for future prescriptions.
